Office Manager. Liane comes to SYNERGY with 20 years of experience in clerical, bookkeeping and office management for small and midsized companies in San Jose, California.
After 10 years as a stay at home mom, Liane returned to the workforce in 2000. She enjoys her relationships with the patients and the clients within the field of holistic health.

APPOINTMENTS
Appointments
To schedule an appointment, please call our office at 859-525-5000 between the hours of 8:30 a.m. and 5:00 p.m. to book your preferred day and time with the practitioner that you would like to work with. If you are unsure of which practitioner would be best suited for you, please ask one of our staff members for a recommendation. They will be happy to assist you.

FORMS
New Client Forms
Some of our practitioners require additional paperwork for the first visit. Please come in 10-15 minutes before your first scheduled appointment time to complete the forms
Alternatively, you can download and complete the forms in advance for the following practitioners:

For all other practitioners, please arrive 5 minutes before your scheduled appointment to complete the paperwork.

POLICIES
Insurance
There are insurance companies in this area that DO provide coverage for acupuncture and other forms of complementary therapy. Call the customer service number on your insurance card to inquire about the specific coverage for your plan.
We do ask for payment at the time that service is provided unless previous arrangements have been made. At your request, we can provide you with an itemized receipt for your insurance company or your HSA plan.
Payment Policy
We accept cash, check, MasterCard, and Visa. We do ask for payment at the time that service is provided unless previous arrangements have been made.
Confidentiality
Patient confidentiality is maintained at all times. No information is given to anyone outside of the office unless it is at your request.
Cancellation Policy
To help keep our office running smoothly, and to allow us to provide sufficient time to all clients, we ask that cancellations be made at least 24 hours before your scheduled appointment time.
There will be a charge for all appointments not kept or cancelled in less than 24 hours.

From the South:
